Use of the JCC/Project Information Sheet & STD-204 is mandatory for any change of physical address: The Appellate Attorney Information Sheet and the STD 204 form should be re-submitted by all panel attorneys with open appointed cases to make a change of physical address. The forms and instructions are available HERE Email submission to APHELP@jud.ca.gov is preferred. (Note: there is no “D” in the email address.)
Don’t forget the projects: A copy of only the second page–Appellate Project Attorney Information Sheet (NOT the STD-204 form) must be submitted to every appellate project that you are a panel member. For submission to CCAP use: eservice@capcentral.org
It is strongly recommended that for privacy and safety reasons you use only postal mail and an address other than your home address for your business address. (See Setting Up Your Office article in the efficiency series for new panel attorneys.)
IMPORTANT! Remember to notify the court, your client, and other parties if you change your address. (Cal. Rules of Court, rules 8.32(b) [attorney info on pending cases].)
- THIRD DISTRICT cases: Truefile a single change of address notice that lists all active case numbers; pick one open case. The clerk will replicate it for each listed case.
- FIFTH DISTRICT cases: Create one change of address notice listing all active case numbers on one notice, but Truefile it in each individual active case.
A Note About Private Mailboxes
A Commercial Mail Receiving Agency (CMRA) is a private business that accepts mail from the U.S. Postal Service for recipients, keeps it for collection (usually a private mailbox, “PMB”), or re-mails it to another location with newly paid postage. Customers are responsible for the expenses associated with the services provided by CMRA stores such as Mail Boxes Etc. Additional information about mail services at CMRAs is available on the U.S. Postal Service’s website HERE
If you rent a mailbox space at a CMRA, the CMRA should provide you with the format to be used for the delivery of your mail. If your incoming mail does not comply with the postal service CMRA regulations, you will not get letters from your clients, documents from the courts, or compensation checks from the State.
