Change of Taxpayer Identification Number

The JCC requires that counsel send:

1. A new Judicial Council of California and Appellate Projects Attorney Information Sheet, Instructions, and STD 204 form.

   Send signed/scanned PDF copy to JCC via email (preferred):
   APHELP@JUD.CA.GOV (Note: there is no “D” in the email address.)

2. OR, send a signed paper original to:

455 Golden Gate Ave., 6th Flr.
San Francisco, CA 94102

Please DO NOT send the STD-204 form to CCAP, as we do not need or want this information.

A change in Taxpayer ID number will not generate a new vendor site ID number; it will stay the same.
