We hope this quick access to our instructions and forms will streamline the appellate-attorney appointment process from your office to ours.

TIP: Take the Notice of Appeal, the Application for Appoint of Counsel, and the defendant Background Information forms with you to sentencing. Assist those clients who need help to fill out all three forms.

File the Notice of Appeal with the Superior Court clerk.

Return the Application and Background Info forms to CCAP.

Please remind your client that if the Application for Appointment of Counsel form is not returned, or in the alternative, a brief filed on his/her own behalf is not timely received, the Court of Appeal will dismiss the appeal. Therefore, it is imperative that your client return the Application form to CCAP as soon as possible if appointed counsel is desired.

Steps to Begin the Appeal

To access the correct group of forms to use, first choose an appellate package for either a Third District case or a Fifth District case from the Trial Attorney Info page.
